We reserve the right to change the medications on this formulary. The presence of a medication on this formulary does not guarantee that a Mercy Health Plans' member will be prescribed that drug by his or her provider. Certain plans do not cover prescription drugs that are not on Mercy Health Plans' formulary or cover such drugs with a higher copayment.

Remember: If your physician prescribes a brand name drug when a generic substitute is available or if you choose to receive the brand drug (with the approval of your physician), you will be responsible for the appropriate co-payment plus the cost difference between the "brand" name and the "generic" name drug.
